VIDEO ORDER FORM

Please fill in all of the relevant information on the form.

Please only order videos for dancers you are associated with. (You can order for multiple dancers but must have permission from parents. Different dancer names can be added at each line)

Each video contains one dance so if you would like to order three different dances you need to order three videos

Your videos will be sent out as a digital download up to four weeks after the event.

A confirmation email with download information will be sent within a few days of your order.​
